RENTAL & REFUND POLICIES

The garments listed here are received by me and are for RENT ONLY.   I agree to return the
rented items on stated return date. An additional $25.00 late fee shall be charged for each
day of use pass the return date up to a maximum of seven (7) days. On the eighth (8th) day,
customer shall be responsible for replacement cost equal to five (5) times the regular daily
rental rate of total rental package or full retail replacement value for missing items in
consideration of merchandise cost and lost revenue. In the event of damaged articles, I agree
to pay on return date all costs of replacements and repairs. I also agree to pay a reasonable
collection fee plus attorney fees in the event a suit is instituted.

In consideration of the non-refundable MANDATORY DAMAGE WAIVER FEE of $10.00, dealer agrees to waive claims against the customer resulting from unintentional minor damage to the rented merchandise.

Waiver fee DOES NOT COVER extensive or malicious damage, loss or theft of the garment.

WITHIN 10 DAYS OF USE DATE, NO REFUND IS GIVEN. PRIOR TO THAT A MINIMUM OF $50.00 WILL BE CHARGED FOR FITTING AND/OR CANCELLATION FEE.

RENTALS MUST BE RETURNED THE DAY AFTER THE EVENT.
